Dekubituset (Ulcerat e shkaktuara nga presioni i vazhdueshem mbi nje zone te ekspozuar te trupit)


Plaget e shkaktuara nga presioni mbi lekuren qe mbulon prominencat kockore (pjese te te dala te trupit qe jane te ekspozuara ndaj presjonit, shtypjes) quhen Dekubituse.
Keto plage shkaktohen si pasoje e imobilizimit, mungeses se levizjeve dhe qendrimit per nje kohe te gjate pa levizur ne nje pozicion.

Presioni mbi lekure shkakton:
- demtimin e qarkullimit te gjakut duke sjelle si rrjedhoje formimin e nje plage (ulcer) dhe
- ndihmon ne rritjen e baktereve (floren normale te lekures dhe mikroorganizma te tjera anaerobike ose aerobike),
- frenon funksionet e leukociteve (sic eshte vrasja e baktereve),
- pengon sintezen e kolagjenit (qe eshte proteine e domosdoshme per mbylljen e plages),
- ndihmon inflamacionin qe shkakton akumulimin e substancave toksike ne inde (amoneve dhe laktatit) duke sjelle si rrjedhoje edemen qe demton rende qarkullimin e gjakut.

Simptoma (shenja) kryesore eshte dhimbja e cila shkaktohet nga infeksioni dhe inflamacioni. İnfeksioni mund te depertoje edhe ne shtresa me te thella duke shkaktuar edhe infeksionin ne kocka (osteomyelitis).

Aplikimet klinike te Terapise Hiperbarike me Oksigjen (HBOT)
HBOT eshte provuar te jete shume e suksesshme duke ju shtuar edhe trajtimeve te tjera ditore sic jane mjekimi lokal i plages, marries se antibiotikeve dhe debridimit kirurgjikal te plages (nese eshte i nevojshem). HBOT:
• Ndalon demtimin e indeve te tjera qe nuk jane demtuar dhe mundeson furnizimin me sasi te larta oksigjeni te indeve qe vuajne nga mosfurnizimi i nevojshem me gjak.
• Nxit dhe ndihmon ne rritjen e indeve te reja.
• Lufton dhe kontrollon infeksionin.
• Forcon pergjigjen e sistemit imun (sistemit mbrojtes te organizmit).
• Perforcon, rrit dhe ndihmon efektin e antibiotikeve.
• Redukton fuqishem edemen e shkaktuar nga inflamacioni.
• Nxit riparimin e indeve kockore te demtuara duke nxitur formimin e indi te ri kockor (osteogenezis).
• Pergatit klime te favorshme ne inde per te pranuar greftet (vendosja e lekures te marre ne nje pjese tjeter te trupit) nese do te jete e nevojshme.
