Plagët Diabetike

Nje nga komplikimet me te zakonshme te Diabetit jane Plaget e Pasherueshme dhe Gangrena gazore.
Faktoret e shumte qe shtojne mundesine tek diabetiket per te patur plage, te cilat me vone mund te infektohen jane:
• Ndryshimet (demtimet) ne enet e gjakut (microangiopathy) - shkaktojne ne inde gjendje hipoksike (mosfurnizimin e mjaftueshem me oksigjen), te cilat sjellin si rrjedhoje demtime ne lekure ne menyre spontane, pengojne sherimin normal te plageve dhe infeksioneve.
• Demtimi i Nervave Periferike (neurupathy) - ne ekstremitetet e poshtme (kembe) duke shkaktuar ndjesi jo normale (i ndiejne kembet te ftohta, te mpira), zvogelojne ndjesine e dhimbjes dhe ne kete menyre ndihmojne ne zhvillimin e plageve traumatike pa u kuptuar nga pacientet.

Te dhenat e para: Pacienti diabetik prej 25 vjetesh, nuk pi duhan, perdor insuline prej 13 vjetesh. Kemba e majte eshte operuar 3 muaj me pare. Kemba e djathte ka 3 ulcera prezente dhe vlera te uleta oksigjenimi te indeve.

Ne rastin e nje plage Diabetike ku:
1. nderpritet furnizimi me oksigjen i indeve nga infeksioni,
2. qarkullimi i gjakut eshte i pamjaftueshem,
3. kemi te bejme me demtimin e sistemit imun,

HBOT rrit presionin e oksigjenit ne plage duke bere te mundur efektet e meposhteme :
• HBOT pakeson dhimbjet duke rritur transportin e oksigjenit ne indet iskemike (indet qe vuajne nga pamjaftueshmeria e oksigjenit) dhe permireson qarkullimin dhe furnizimin me gjak.
• HBOT pershpejton sherimin e plageve duke nxitur rritjen e indit te ri dhe te shendetshem (formimin e kapilareve te rinj qe quhet neoangiogenezis)
• HBOT redukton infeksionin - duke vrare ne menyre te drejtperdrejte bakteret -duke rritur kapacitetin e qelizave te bardha (leukociteve ) ne lufte kunder infeksionit dhe ne eleminimin e mbeturinave (gjate nje infeksioni kemi mbetje si qeliza te vdekura) te formuara gjate luftes qe ben organizmi kunder infeksionit.
• HBOT rrit efektshmerine e disa antibiotikeve.
• HBOT redukton edemen (fryrjen ose sic mund te themi ndryshe enjtjen ne zonen e infektuar). Edema ndihmon -ne perparimin e infeksionit dhe te pasojave te tij si dhe -ne demtimin e qarkullimit te gjakut si rrjedhoje demtimin ne teresi te zones se prekur.
• HBOT pershpejton fuqishem formimin e indit te ri kockor (osteogenezis), ne kete menyre ndihmon ne zgjidhjen e infeksionit kockor (osteomyelitis) dhe ndihmon ne shkeputjen e indeve kockore te vdekura pas infeksionit (aktiviteti osteoklastik).
• HBOT fuqizon sistemin imun dhe ne kete menyre parandalon infeksionin ne pergjithesi.

Tedhenat pas HBOT: Para dhe pas 9 seancave HBOT: Te gjitha ulcerat te mbyllura. Vlerat e oksigjenimit te indeve jane siper normales.
